DR. STEFANOS G. PANTAGIS, MEDICAL DIRECTOR
Dr. Pantagis is the founder and Medical Director of Geriatrics & Longevity Treatment Specialists. He graduated from UMDNJ Medical School and specializes in Family Medicine and Internal Medicine. He is a Board-Certified primary care physician with vast knowledge in resolving complex medical problems and making recommendations to improve treatment of patients in hospitals, long-term or acute-care rehabilitation facilities, nursing homes, home care, and palliative care. Dr. Pantagis is a trusted, patient-focused physician with a long history of serving patients and their families throughout Bergen County. He is affiliated with Englewood Hospital and Medical Center, Holy Name Hospital, Hackensack University Medical Center and Hackensack University Medical Center at Pascack Valley. He also works closely with several Hospitalists at Valley Hospital.
MARY SIMMONS, APN, MSN
Mary is a Nurse Practitioner and has worked extensively with adult and geriatric population for over 25 years. She has a combination of over three decades of home care and community health experience and has been a nurse for more than 40 years. She graduated from St. Peter’s University’s Adult Nurse Practitioner Program in 2014.
TAMAR SCHICLY, APN, FNP, ANCC
Tamar is a Family Nurse Practitioner with experience in Urgent Care and Medical-Surgical step-down units at Lutheran Medical Center in Brooklyn, and New York Presbyterian Weill Cornell in Manhattan. She received her Nursing degree from SUNY Downstate School of Nursing, and then went on to obtain a Family Nurse Practitioner degree from the same university.
JACKIE BAUMRIND, MS, RD, CD-N, LNHA
Jackie is a Registered Dietitian with a Bachelors Degree in Business Management from Boston University and a Masters Degree in Nutrition and Education from Columbia University in New York. She has worked with the adult and geriatric populations for almost two decades in all aspects of nutrition, including Medical Nutrition Therapy, Disease Management, Health and Wellness, and Weight Management. Jackie has been on staff at top hospitals and healthcare facilities in both New York and New Jersey. She is also a Licensed Nursing Home Administrator and has managed sub-acute rehabilitation care facilities in Northern New Jersey.
LAUREN CARUCCI, APN, FNP
Lauren graduated from William Paterson University’s accelerated BSN program in 2005. She has worked as a nurse for the last 12 years gaining experience in Emergency Medicine, CHF/Cardiology and Intensive Care and has spent most of her career within the Hackensack University Medical Center system. In 2012, she obtained her Master’s Degree from Farleigh Dickinson University as an Adult Care Advanced Practice Nurse.
DANA HAYES, LCSW, CCM
Dana is a Licensed Clinical Social Worker with over 13 years of experience in Medical Social Work. She holds both a Bachelors Degree in Psychology and a Masters degree in Social Work and is Licensed in the State of New Jersey as well as holding her Certification in both Case Management and Clinical Social Work Supervision. Dana has worked extensively with patients dealing with all spectrums of care including chronic, terminal and mental illness as well as providing death/dying and grief counseling. She combines supportive counseling with her genuine care of all those she meets to ensure the highest quality of life. Dana brings a unique kindness and sensitivity to individuals suffering from chronic illness, comforting patients and families, and has a thorough knowledge of advanced care planning options and community resources.
ABIGAIL PERRELLI, MSW, LSW
Abby Perrelli, MSW, LSW is a Licensed Social Worker with over 19 years of experience as a social worker in various settings and populations such as children and families, adoptions and mental health. Over the past 11 years she has been focused on medial social work in the hospital and long term care settings with the last 9 of those years focused on home hospice. She has lead support groups on grieving the loss of a loved one and for those diagnosed with Parkinson’s Disease. Her career has lead her to work in diverse settings in four different states in rural, urban, and suburban environments, often times with little or no resources. She has also served a term as a school board member. With all of these different populations that she has had the pleasure to work with, her passion is the geriatric population and finds joy in utilizing all of her experience and skills to helping patients and families.

Jessica has been a nurse for over 10 years, specializing in wound care and geriatrics. She has worked in a variety of outpatient settings such as sub-acute and long-term care facilities, and assisted living. Jessica is our office manager as well as our wound care specialist.
TALONNIE WHITE – Office Staff
Talonnie has worked in healthcare for over 10 years with experience in various healthcare facilities. Her primary focus is overseeing medical records and care team coordination.
BRIDGET ALLMAN, LPN – Office Staff
Bridget is an LPN with over 20 years experience in healthcare, and is passionate in providing exceptional care to a diverse patient care population.
KARLA INCHAUSTEGUI – Office Staff
Karla has a bachelor’s of science in Business Management. She is enjoys helping others and was a peer leader in the Peer-to-Peer Program at William Paterson University where she mentored and guided incoming and transitioning students.
LUIS XHIXHABESI, LPN – Office Staff
Luis is an LPN with over 8 years experience in healthcare, working in assisted living, sub-acute, ventilation units and long-term care. He has worked closely with the Alzheimer’s/dementia population and has a solid awareness of their medical and psychological needs. Luis has also assisted in teaching phlebotomy and standard care practices to students.
